Blister Pack is Filled Improperly
- ALL compliance pack errors detected by the pharmacy technician must be documented by the pharmacy supervisor as per the protocol for reporting and documenting compliance pack errors and near misses. This report must then be sent to the pharmacy manager for review.
- IF there is an error in the compliance pack, the pharmacy tech will return the compliance pack to the pharmacy assistant so that any corrections can be made.
- A note will accompany the compliance pack which notes the error in the compliance pack to be addressed.
To make any modifications to the closed compliance packs, follow these steps:
1. Using a scalpel cut out the paper backing from the blister pocket that has an error
2. Make any necessary corrections to the compliance pack. Any drugs which need to be removed should be removed with tweezers and returned to the open stock medication bottle from which it came.
3. Once the error has been addressed, seal the open pocket of the compliance pack using the compliance pack correction tape
Medication is Unavailable for Required DIN
- IF dispensing a medication from a stock medication bottle and there are not enough units to fill the compliance pack order the pharmacy technician must be notified of this medication shortage.
- IF a particular DIN called for in a compliance pack order is unavailable in the pharmacy, but other brands of the same medication are available:

- IF no drug substitution is approved, the pharmacy technician must be notified of the medication shortage so that an order can be placed to bring the required product into the pharmacy.

To note a shortage of the medication adhere to the following procedure:
1. Write a note which includes the following information:
- Pharmacy Client (Example: Clarke's)
- Patient Name
- Product DIN
- Quantity required to fill the compliance pack
2. Bring this note to the pharmacy technician
3. Affix a separate note on the compliance blister pack which is awaiting the drug order with the same information
4. Nest the blister packs with the drugs inside within one another and put aside at the compliance packing station
5. As new product arrives at the pharmacy, review the compliance pack notes and collect the medication required to complete the fill
6. Make the addition of the missing medication to the blister packs as required
7. Seal the compliance pack and affix the labels in accordance with the protocol for filling compliance packs.
